I have lived in Italy for over a decade and October is my favorite month. The summer heat finally breaks. The streets feel like they belong to locals again. You get real autumn light, golden and soft, on the same monuments that suffocate under July selfie sticks.
Weather and what to pack
Rome and Florence sit at 18, 24°C in early October, dropping to 12, 18°C by the end. Venice is cooler and damp. Naples and Sicily stay warmer, often 20, 26°C. Pack layers. A light jacket and a scarf will save you. Umbrellas matter more in the north. For example, Milan averages 9 rainy days in October.
Crowds, prices and what is open
This is the sweet spot. Major museums stay open with shorter queues. The Colosseum waits drop from 90 minutes in August to under 25 minutes by mid-October. Hotel rates fall noticeably. A standard double in central Florence that costs 280 EUR in August costs around 180 EUR in mid-October. Restaurants still offer al fresco lunch. Many coastal ferry routes stop by late October, so check schedules for Capri or Cinque Terre.

Events worth your time
Truffle fairs happen across Piedmont and Umbria all month. White truffle season starts in Alba, where a single gram can cost 8 EUR, but the scent is worth every euro. Also check out the Eurochocolate festival in Perugia, usually the third week of October. It is crowded with families but the free samples are honest.
A few warnings
Skip the Amalfi Coast and Cinque Terre in late October. Many restaurants close for annual holidays. The ferries shrink to a skeleton schedule. Do not expect beach weather. Instead, head inland. Bologna, Lucca, Orvieto, and the hill towns of Tuscany and Umbria are perfect this month. The light at 4pm in San Gimignano is absurdly good, and you will share it with fewer than a dozen other people.
Simple rule: come in the first three weeks of October, skip the last week. You will get the best of autumn without the rain of late month.
